Release of Information: Insurance Verification and/or Claim History Report

All requests to release information must be accompanied by a properly executed release. The release is a fillable PDF that may be completed online or printed and manually populated, must be printed for signature (wet signature), scanned and saved as a PDF, and returned to the Insurance Services team via the below option for processing.

Named Insured on the policy.
Your email must match the email we have on record associated with your AAHP-RRG policy. If not, this will delay and possibly void this request.
Specific Recipient details should be populated on the Release of Information form that is to be attached with this submission.
